At Midland Children’s Rehabilitation Center we strongly believe that early therapeutic intervention is important. The demand for pediatric physical, occupational and speech therapy as well as dyslexia tutoring in our area is constantly increasing with each passing day. To begin the intake process, please, follow the link below to the center’s intake forms.

Once you complete the MCRC intake forms and return them, it’s possible that your child may be put on a waiting list. In the event that happens, we encourage all families to contact the facilities listed below to see if they are able to process your request for an evaluation and treatment sooner. We do this because we want your child to begin receiving the benefits from treatment as quickly as possible.

If you choose for your child to receive therapy at another facility prior to being scheduled at MCRC, please, let us know so that we can remove you from the waiting list. It is not our policy to provide treatment to a child that is already receiving services from another provider. Thank you for the opportunity to care for your child.
